DSHS DIRECT CONTRACTING

Have you ever had to bill DSHS directly for your interpreting services?  Here's some useful information about this process.

The DSHS employee should be able to provide you with the proper forms to fill out, and indicate where to send them. If s/he does not have them or know where to get them, s/he should consult her supervisor. How much should I charge? Does DSHS pay for mileage? You will find answers to this questions in the DSHS Payment Policy document attached below.

You will need to fill out at least two forms if not four:

1) DSHS Form 17-123 (see attachment below)

2) 19-1A form - Vendor's Certificate (see attachment below)

3) W-9 if not on file already (see attachment below)

4) WA Statewide Payee Registration Form to receive direct deposits from the state (see attachment below)

Did you drive farther than 30 miles each way? That should be indicated on your 17-123 form and be accompanied by a standard I-net mileage print-out showing mileage both ways.

If the DSHS employee can give you the phone number for the field office on which your assignment depended, you can call there and ask which forms you should have and where to send them. (If not, type WA DSHS Field Offices in your search bar.)  Keep photo copies and follow up. If the Field Office people are uncertain, ask for the central payment phone number in Olympia and don't be afraid to call there. Have your photocopies and a history of where and when you sent the originals ready in front of you.
